Colfax Small Business Grant Program Application Checklist

 

This document will not be accepted in place of the grant application. Please use this as a tool to gather the required information in advance. Applications will be accepted online from February 8, 2021 to February 19, 2021.

 

SMALL BUSINESS APPLICATION

 

  1. ELIGIBILITY
  • Review the small business eligibility requirements HERE.

 

  1. BUSINESS INFORMATION
  • Annual gross revenue for 2019
    • Annual Gross Revenue is also known as Gross Receipts or Sales. This is your total revenue before expenses. This is not your income or net profit.
    • If you are a sole proprietor, your Annual Gross Revenue for 2019 should be listed on line 1 of the Schedule C you normally file to report your business income for federal tax purposes.
    • If your business was established on or after January 1, 2020 and before March 1, 2020, what is your projected Gross Revenue for 2020?
  • How many employees does your business have?
    • Calculate the number of full-time and part-time employees equivalent to a 40-hour workweek as of March 1, 2020. You can also check your paycheck provider’s IRS Form 941 for the number of FTEs.
    • DO NOT include independent contractors.
    • If you are a seasonal business, please include employees at your peak time.
    • The business owner is considered ONE employee.

 

  1. SHORT NARRATIVE QUESTIONS
  • Describe how the COVID-19 pandemic and shutdowns have impacted your business in terms of closures, lost revenues, layoffs, lost inventory, etc.
  • Please provide a brief summary of how you anticipate using the grant funds, if awarded.

 

  1. DOCUMENTATION
  • A copy of a current and valid City of Colfax business license
  • A completed W-9 form (max file size 5 MiB)
    • This will help expedite the processing of your grant application. Download a copy of this form from the IRS website: English W-9 or W-9 en español
  • Photo/screenshot or scanned copy of a driver's license, Real ID, passport, or other photo ID to verify your identity.

NON-PROFIT ORGANIZATION APPLICATION

 

  1. ELIGIBILITY
  • Review the non-profit organization eligibility requirements HERE.

 

  1. ORGANIZATION INFORMATION
  • Service area, mission, and program focus
  • Number of full and part-time employees as of March 1, 2020 (pre-COVID), excluding independent contractors

 

  1. SHORT NARRATIVE QUESTIONS
  • Describe how the COVID-19 pandemic and shutdowns have impacted your organization in terms of closures, lost revenues, layoffs, lost inventory, etc.
  • Please provide a brief summary of how you anticipate using the grant funds, if awarded.

 

  1. DOCUMENTATION
  • Current operating budget (budget vs. actual)
  • A completed W-9 form (max file size 5 MiB)
